A medium rare burger can be safe to eat if it is cooked to an internal temperature of 145F (63C) and held at that temperature for at least 3 minutes. This is the recommended temperature by the USDA to kill harmful bacteria like E. coli.
I like to start on medium high to brown outside (1-2 minutes per side), then finish on medium low to desired level of how you wish your burger cooked. After browning, it also works well to remove to a baking sheet and finish in the oven at 300 degrees. With either method, keep a close eye on internal temperature to achieve desired finish of the cooked burger. (125 = medium-rare, 130 = medium, 135 = medium-well).
The recommended internal temperature for a medium rare burger is 130-135F (54-57C).
It depends on what type of meat the burger is. If the meat meat is one with holes in it (like ground beef), it must be well done. Unlike steak, meat with holes in them must be cooked well done to kill all the bacterial in them.

Kobe beef should be cooked either rare or medium rare. It is up to the person who is eating it to decide which they like. However, it should not be cooked more than to medium rare.
