heavy workout about 3-4 hours...low calorie intake....high protein,and good carbs...and unsaturated fat.....lots of fruits and lots of water...this would do it......

because once you lose the weight lots of the time you start gaining back your bad eating habits. or if you lose weight too fast you will regain it
I doubt that you can starve yourself for a month as your body will not allow it. It all depends on how much you weigh now and how active you are.

Don't think there really is one, it depends too much on how harsh their diets are and on how good they are at following them
