The valence electrons are either lost to another atom or the sodium atom gains valence electrons, it really depends on if what the sodium atom is bonding with has a lot or a little of valence electrons. The structure doesn't change though, just the number of valence electrons change.
The nucleus is never changed when an ion is formed.
