The loudest noise ever was the eruption of a volcanic island in the Pacific Ocean. The island of Krakatoa in Indonesia that erupted in 1883. The island blew a apart and caused a major tsunami.

I'd imagine that the blast wave noise from an atomic weapon would be the loudest man-made noise, but I can give no figure. The largest of these was a hydrogen weapon named Tsar Bombe, and had a yield of 50 Mega tons.

Other than that, probably the Saturn V rocket - actually a set of 5 rocket engines would also be in the running, and has a more sustained noise.

The Thunderscreech airplane, XF-84H produced sonic blast waves from its supersonic prop that caused real problems, and could be heard 40km distant.

A Space Shuttle launch generates over 200 db. (more than a few atmospheres of over-pressure).

An explosion at Norfolk Naval Facility in 1943, was heard 30 km distant.

[In Port of Texas City, a1947 explosion of a shipload of NH3NO3 gave a demonstration of this powerful and cheap explosive. Made a hole in the harbour. Was heard 150 km away in Louisiana. ]
