I guess it could be wrong_that is_ allocate authority and responsibility on the chart differently from where it works. But let's assume that we're talking about an accurate organizatuin chart. Well, even the accurate ones don't reflect the informal chart. You know, John in accounting is in the carpool with Fred in Marketing so almost by osmosis each picks up information and insight that effects (affects) their performance. Maybe the information leads to wrong decisions and the organization suffers. The chart can be too strict and discourage cross fertilization of ideas. The "going through the chain of command may be too lengthy and not time sensitive. Fred sees an electricity danger in the plant, he reports it to his boss in production who subsequently reports it to the head of safety who sends an email to plant maintence, who sends it to the supervisor of electricity who sends a man or woman out who carries off the dead bodies that piled up AND fixes the problem. Oh and chain of command discourages reporting, for instance, bosses who are performing poorly or sexually harassing. If you report it you are almost by definition going outside the organization chart. I mean you can't report the bad behavior to the boss DOING it.
