The state averages about one shark attack per year, with fatalities being quite rare. Most encounters result in non-fatal injuries, and Hawaii remains a popular destination for ocean activities despite the risks.

This is difficult to answer because it is so subjective, but many might say that it was following the sinking of the USS Indianapolis in July, 1945. Over 500 sailors made it off the ship, but died in the following days from exposure, drowning, and shark attacks. It is unknown exactly how many died from shark attacks.
