Yes. His effect does not say it it limited only to face-up monsters.

But if it was equipped to a monster, and an effect destroyed it (or destroyed it and the monster simultaneously) then that effect will be applied.

No, because it never gets flipped face-up. You can activate Raigeki Break to destroy an opponent's face-down Man-eater Bug without any monster on the field getting destroyed.
Is the effect of Retilianne Medusa permanent?
The effect does not wear off at a specific time, not even if Reptilianne Medusa is destroyed. The effect will be applied to the affected monster for as long as that monster remains face-up on the field. If it is flipped down, then up, or removed from the field, the effect is removed too.

If the face-down monster is tributed, then no, any Flip Effects it has will not activate. Under normal conditions therefore, you should flip the monster first, allow the effect to activate/resolve, then tribute the monster for whatever. But you are out of luck if your face-down Flip Effect monster is tributed by the opponent using Soul Exchange, for example. Because it never 'Flips' (even though it goes to the graveyard face up) the effect will not activate.

The Equip happens on resolution of an Ignition Effect. Book of Moon can be chained to that. The target monster will flip face-down and then the Equip effect will resolve without effect because it can't equip to a Set monster. Inzektor Hornet will not move from whatever zone it started in.

No monster can activiate its effect while face down unless it specifically says so; only face up.If you are referring to monsters who say their effects activate when Flip Summoned, that means you must flip the monster up manually during your Main Phase after it is set face down.
