dumbledore dies because he gets poisened by one of voldemorts horucruxes so dumbledore asked serverus snape to kill him and he did so he died. snape made an unbreakable promise to kill dumbledore and in the last book it says that dumbledore allowed snape to kill him. snape wouldn't have made the promise if he hadn't been allowed to.

Before Harry dueled Voldemort, he, Ron, and Hermione saw Severus Snape brutally murdered by Voldemort whise hiding in the secret passage to Hogsmeade (Harry Potter and the Prisoner of Azkaban). Voldemort goes to Hogwarts to find Harry. Harry then comes out of the passage before Snape dies, and collects Snapes memories. He then goes back to Hogwarts, where Voldemort has given Harry one hour to come to him and turn himself in. Harry puts the memories in the Pensieve and sees how arrogant his father was, and that Dumbledore's death was actually planned between himself and Snape. He then learns in one memory that he is actually a Horcrux, too. With this information, Harry finishes Voldemort. Then he honored Snape by naming his second child Albus Severus, Severus being Snape's first name.

Yes. Harry goes to Voldemort because after viewing memories of Dumbledore and Severus, he finds out Dumbledore planned for him to die, and for Voldemort to kill him. Once Voldemort kills Harry, Harry is given the choice to die or come back and fight. Harry decides to go back and fight, which seems as though he died and then came to life.

Snape is not a muggle. He is a half-blood, hence the name the 'Half-Blood Prince.' His mother's maiden name was Prince, and his father was a muggle. Snape reveals who he is during the Battle of the Astronomy Tower, shortly after Dumbledore was killed. I don't remember the exact chapter.
Although Dumbledore died in the Half-Blood Prince, he returns in the Deathly Hallows when after Voldemort kills Harry Potter, and comes to him, giving him a choice to "move on" or come back and fight.
