In the manga tuxedo mask gives Sailor Moon an engagement ring and at the end of the series they get married. In the anime tuxedo mask gives sailor moon a ring before he goes off to school, and kisses her goodbye and eventually come back. So in both they end up together.

They remained broken up until near the end of the season, when the truth about Mamoru's dreams was revealed, along with their source: his future self, King Endymion. After that, they got back together and remained that way through the final season, when Mamoru gave Usagi a promise ring (an engagement ring in the manga) before he departed for Harvard in the United States.
In the anime, Mamoru/Damien 'died' via Galaxia when she attacked the airplane he was on during the beginning events of the Sailor Stars. His Star Seed (the golden one in the anime) was stolen from him, thus rendering him in a dead coma-like trance. After the events in the anime, his Star Seed is returned to him at the end, waking him up and successfully 'reviving' him. Not sure about Manga.
They reunite in English dub episode #70: "Promises Fulfilled," episode #77 in the original Japanese, "The Same Thoughts! Usagi and Mamoru in Love Again!" and remain together from that point onward, despite Mamoru occasionally get brainwashed, kidnapped, or outright killed by the enemy.

Depends. Most catchers who wear a hockey style mask do not remove it to catch a pop-up. If you use an old-school mask, you wait to throw the mask until you have an idea as to the direction the pop-up is heading. You don't throw the mask right away since you do not want to end up tripping on the mask if you have to backtrack.
