Most commonly this occurs in the trachea (cigarette smoke) and the result is that the mucus that lines the trachea is not carried up to the esophagus to be "drained off". The end result is that the inhaled bacteria remains in the airways where they can grow and cause infection.

There will be stagnation of the mucus in dependant part of respiratory tract. This will lead to infection, leading to pneumonia.
