The terms of surrender in the Persian Gulf War, specifically for Iraq, were outlined in UN Security Council Resolution 687, adopted in April 1991. Iraq was required to withdraw from Kuwait, accept the destruction of its weapons of mass destruction, and allow for inspections by the UN. Additionally, Iraq had to pay compensation for the damages caused during the invasion of Kuwait. The ceasefire officially took effect on April 11, 1991, following Iraq's acceptance of these terms.
The UN drafted the ceasefire agreement which ended the fighting. (Iranian-Iraqi relations would not resume until 2003 with the fall of the Saddam Hussein government in Iraq.)
The United States and Iraq are currently on good terms, but they are not members of any sort of alliance.
There aren't three reasons why it ended. There is really only one. The United Nations proposed and re-proposed and re-proposed a ceasefire. After eight years of fighting, both sides were exhausted and decided to accept a United Nations negotiated armistice.
A ceasefire is a temporary stop to a war when both sides agree to suspend fighting. It is a signal or an order to stop fighting.

NO. The United States (as well as every other world power) is not considering a ceasefire with Islamic State (Daesh - داعش). The ceasefire recently achieved was between the Assad Regime and the Russian bombing raids that have supported them on the one hand and the elements of the Free Syrian Army (al-Jaysh as-Suri al-Horr - الجيش السوري الحر) on the other. A ceasefire with Islamic State or Al-Qaeda Islamic Maghrab (AQIM) would be unreasonable.
Ceasefire Canada was created in 2003.

North Korea is the only one currently. No peace agreement was ever reached - only a ceasefire. We're not at war with Afghanistan - just with the Taliban, and Iraq has been reduced to a policing and support operation.
