The first ballistic missiles to be deployed was the German V2 rockets towards the end of WW2, in that they were fired from Germany and aimed at London and the surrounding area.

Happily, although nations subsequently developed the capability of deploying ballistic missiles to travel between continents with anything up to and including nuclear warheads, they have always managed to pull up short of actually doing so - and the general disarmament of 1990 onwards has reduced the threat.

Of course, there are always rogue regimes (Iran and N Korea immediately spring to mind at the moment), who seek to develop such weaponry for their own ends, but none have so far actually deployed against their enemy.
