If any one phase fails , the motor still continues to run but the temperature raises rapidly, due to high current. The motor winding can burn out, can trip off due to overload.
Hence a device called phase failure relay is connected. It constantly reads all the 3 phases and cuts off the main supply in case any one phase fails.
