The exact mileage per year is the odometer reading at the end of the year minus the odometer reading at the beginning of the year. If you don't have the odometer reading from the beginning of the year, the next best thing is to subtract an older odometer reading from the reading at the beginning of the year, then divide the difference by the number of years between readings. Without any odometer readings from the past, the best you can do is to calculate the average annual mileage over the life of the truck, which is the current odometer reading divided by the truck's years of service.

Yes. It will work without any modifications. However, local laws may prevent you from being able to do this legally. If the odometer is built into the speedometer, the new odometer must read equal to or higher mileage than the odometer you are removing. Not following such laws may result in fraud charges if you decide to sell the car in the future.
There are a few places one can go to read about the year 2012 in review. The Time Magazine has such a feature once a year in the United States while in Canada there is a magazine called The Macleans Magazine which does the same.
Distance. If you drive away from the house one morning, drive around town all day, and return home in the evening, the odometer properly registers several kilometers added, but the day's dislacement is zero. Even better: If you've owned the car for a year, the odometer may well display 20 thousand kilometers, although the car sits parked in the same garage today as the one where it sat on the day you bought it.
You need a scan tool, or cycle the key on,off,on,off,on and depending on the year either count the flashes of the check engine light or read the code in the odometer.
Depending on the year of your car, you may be able to take it to the dealership and have the computer chip in your car read. These machines typically know how far you have gone even if your odometer is no longer functioning properly.

It would depend on the year, make and model vehicle.
The intermittent odometer in model year 1999 - 2003 Ford Expeditions (also same model year Excursion, F150, F250, F350) is caused by cracks in solder joints on a circuit board that powers the odometer. You can fix the problem yourself in about an hour with the instructions available at http://www.odometer-repair.com .
From a 2004 survey, Transport Canada says 16,000 KM for light vehicles. See the link for more stats and info. http://www.tc.gc.ca/pol/en/report/anre2005/7D_e.htm
If you open the odometer, there are two small plastic gears. One is connected to the speedometer, the other to the odometer. The speedometer gear drives the odometer gear. In all likelihood, one of these gears is missing a few teeth. I have the same problem with my Ramcharger, same year.
