Depends on your situation, if you are married to a US citizen, if you are applying to become a student, have a job offer etc.

Personally, I am a Canadian married to an American and it has cost me about 3 thousand so far. The fees to the government are about 1500, you have to get a medical exam which ranges from 300 to 700 generally depending on where you are coming from and what they require and I found a lawyer who took my case for only 1000 which is very cheap. I would not suggest trying to do it yourself without a lawyer. There is a really good reason that Immigration lawyers charge so much, it is a complicated law and without a lawyer you risk wasting tons of time and money. Good luck.

I think it varies depending on the country. Mine for Russia ran over 200.00. The thing about a visa is that there is a time frame to get it. I couldn't file for my Russian one more than 30 days before, but had to have it before I left. There are visa services that will take it to the embassy for you and mail it back stamped on your passport. Yes,you have to send your passport to them. I thought about going to the Russian embassy in San Francisco myself, but the visa service was the best way to do it.

If you are a US citizen you don't need a visa for Canada. All you need is a passport and that runs about 70.00 to file. It is countries like Russia where you need a visa.
