Worlds are given to you in sets of cards. These cards are used to unlock a portion of your memory and visit various Worlds. However, there are certain Worlds that are eventually gained separately, not in the form of sets. You must progress through the story to unlock these Worlds. Do you need to play Kingdom Hearts Chain of Memories to understand Kingdom Hearts 2?
It depends. If you prefer to know every bit of the story, then I would strongly recommend playing Kingdom Hearts Chain of Memories* before playing Kingdom Hearts II. It is not completely necessary, however, due to the constant recap and explanation throughout the series.
Most would consider Kingdom Hearts Chain of Memories a filler, or a side-game, but it does have some important content, such as the introduction of Organization XIII, as well as the reason why the characters are where they are at the start of Kingdom Hearts II.
*Note: If you don't have a Gameboy Advance, you could just as easily get Kingdom Hearts RE: Chain of Memories (a remake for the PlayStation 2). This will help explain the story just as well, if not better, than the GBA title.
