Not in MLB. A batter may bat from whatever side he wants to on whatever pitch he wants to. The only restriction is that the side the batter is on when the pitcher steps on the pitching rubber is the side the batter must bat from for that pitch. A switch hitter could alternate sides for each pitch if he so desired. Rules concerning the batter can be found in Section 6 of the Official MLB Rules.

Baseball was not in the "original" Greek Olympics because it wasn't invented yet. It also did not appear when the games were in Greece in 1896. However, in 2004 when they were in Greece, baseball was a sport there. Furthermore, the Greeks had a team, as the host country they were entitled to that right. However, since baseball is not popular in Greece, the IOC ruled that anyone who had a parent or grandparent who was a Greek citizen could play for the Greeks, thus allowing players such as Clay Bellinger to play for the Greek National team.

Yes, it debuted as an official Olympic sport during the 1992 Barcelona Olympic games. Both baseball and softball have been eliminated as an Olympic game after the 2008 Games in Beijing. There will be no baseball or softball at the 2012 Games. There is a possibility they could make a come back at the 2016 Games but that has not been decided yet.
Deion sanders is famous for playing in both the NFL and MLB. He eventually had to end his baseball career though.
I believe that the 1942 Olympics did not exist. At that point, there were 1940 Summer and Winter Olympics and 1944 Summer and Winter Olympics, etc etc.
