Technically no American president has been impeached and subsequently removed from office. Andrew Johnson was impeached, but acquitted. Richard Nixon resigned before his likely impeachment. Bill Clinton was impeached, but not removed from office. Presidents impeached: * Andrew Johnson: impeached by the House but acquitted by the Senate * William (Bill) Clinton: also impeached but acquitted. Therefore, there have been two impeached, none removed.

Andrew Johnson was impeached in 1868, primarily for violating the "Tenure of Office Act" - Johnson did not like Secretary of War Stanton, and Congress passed the Act to prevent Johnson from firing him. When he fired Stanton anyway, Congress impeached him. In the subsequent trial, the Senate failed to achieve the 2/3 vote needed to convict, so Johnson was acquitted.

Bill Clinton was the second one impeached, at the end of 1998. He was under investigation for possible illegal activities in the so-called "Whitewater affair". Ken Starr, the investigator, found evidence that Clinton had had an extramarital affair with one of his interns. Clinton denied having the affair; he was impeached for perjury and obstruction of justice when it was found that he lied when he denied the affair. He too was acquitted of the charges.

Abraham Lincoln should have been impeached.
