Article I of the U.S. Constitutuion specifies that the House of Representatives "...shall have the sole Power of Impeachment." Article I also states that the Senate "...shall have the sole Power to try all Impeachments." And "When the President of the United States is tried, the Chief Justice shall preside." In other words, the House of Representatives makes the decision to Impeach the President. It then presents the Articles of Impeachment to the Senate where the case is tried. The Chief Justice of the United States presides over the trial, and the Senators vote on whether or not to convict the President. Conviction requires a 2/3 vote.
No. The impeachment process is the only lawful way to remove a president from office. Of course, he could voluntarily resign under the threat of impeachment as did Richard Nixon.
A liar should remember his lies, otherwise he might contradict himself later on. For example, should he tell someone he's a vegetarian, he might later eat meat in front of the person.

Responsibility lies (with Mary, for example). The word "lay" is used to indicate responsibility as well : "He wants to lay the blame on me."
Presidents are impeached by the House, but tried by the Senate. Impeaching someone is essentially charging them with a crime. The Senate then hears the trial and serves as the jury. The trial in the Senate is presided over by the Chief Justice of the Supreme Court.
yes the author has a responsibility to be honest because if they weren't then there wouldn't be any point in writing a book of lies
