Anything that you download from the internet and install on your computer can give you viruses/spyware. Often games aren't packaged with spyware, but can be affiliated with it (you know those check boxes at install time? toolbar this? search engine that? beware!).

That said, internet games leave your computer vulnerable to hackers by opening a port on which to run (a port through your firewall). Once this happens, a hacker *could* get in. Not that it happens all the time, but it happens. Same, actually for toolbars...
