You have to have two sims in love (in order to have a child it has to be a male and female couple) and once their relationship is high enough you can click on a double bed and select "Try for Baby". Once they are done with their Woohoo select the female sim and click on a toilet and do a Pregnancy Test.
You will either get a notification saying you are eating for two or one saying that you were unsuccessful. If your sim didn't conceive, she will get a sad moodlet.
