Treasure chart #26 is located at 6 eye reef. After you clear the Forsaken Fortress for the second time, you come here and destroy all the wall-mounted cannons. Then a chest should appear. So now you would walk on the outer wall of the reef and use the wind waker to change the direction of the wind and then use the deku leaf to fly over there! Then you can use that chart to find the big octo chart. Hope this helps!
