We speculate as to the reason why this happens, with the general concensus being that a mother will kill and sometimes eat her newborn if she suspects the baby is very ill (less chance of survival) or if she had a litter that was too big for her to manage (not enough food in her area to sustain them)

The eating part is probably a defense against predators. A predator finding the dead kitten will suspect that there are likely other kittens in the area, so removing the evidence by eating the dead kitten may be a way to help protect the living ones.

Yes, it can happen. My old cat once had kittens when we weren't home, and when we arrived she had eaten them. It was a bloody mess, but it seems that they were stillborn. I don't know if a mother cat will eat her kittens alive, but generally a mother will only consume her babies if there was something wrong with them (i.e. Stillborn), or thought her and her kitten's lives were in danger. However, for healthy pet cats this occurrence is not very common.
Sometimes after the female cat has given birth, the male will try to eat it so he can get the female to have another kitten. However, the chances of this happening in domestic cats is rare.

The mother cat shouldn't eat a healthy kitten under normal circumstances. However she might if its ill/too small to survive/dead. She might eat it if she's too hungry, but I don't think she would. Usually just to stop it from suffering or if its dead. If she eats more kittens then its a problem. Sorry I don't know more. Hope it helped at least a little.
