check the kennel club the dog is registered in. www.akc.org is the American Kennel Club's website. If the dog's from that kennel club, you follow the links to the pedigrees. You should be able to order it if you have the registration information for your dog.
If you have not been provided AKC registration papers for your puppy from the breeder or you have acquired an older dog you believe to be a purebred you can register him/her with an AKC Indefinite Listing Privelege (ILP) registration. The dog must be neutered or spayed for this but can compete in AKC competitons other than conformation showing. You can find requirements and forms at AKC.org.
It can be difficult to predict the exact appearance of a mixed breed puppy when it is fully grown because their physical characteristics can be a combination of those from different breeds. However, you can get an idea by looking at the traits of the parent breeds.
If you cannot find your puppy's registration papers, you can contact the organization or breeder from which you purchased your puppy. They may be able to provide you with a duplicate copy or help with the registration process. Alternatively, you can also look into registering your puppy with a different kennel club if needed.

If its a purebreed the usually come with papers if not ask to see the mom and dad.
If the puppy was purchased at a pet store, contact the store to see why the papers were not provided. If there is a problem that cannot be resolved, contact the Better Business Bureau in your area. If you have received nothing in writing, only a verbal promise that pedigree papers would be provided, there is nothing you can do except to rely on the honesty of the person who sold the puppy. If you can prove that papers were to be supplied and you have not received them within a reasonable time, the final step would be court action if you feel it is worth the expense of trying to obtain the papers. It is also possible that no pedigree papers exist for your puppy and the promise by the seller was fraudulent.
A puppy w/o papers might come from a puppy mill, where dogs are bred with no concern for the mother's health, the genetic suitability of the parents, or the nutritional needs of the dogs. They are often sickly. Or maybe not a puppy mill but simple cluelessness. Doesn't change the outcome much.

Absolutely no! Anyone can breed a dog with CKC papers, just like with AKC papers. It is up to you the potential puppy buyer to check out the person you are purchasing a puppy from. Do NOT buy from pet shops as most get their pups from puppy mills. Purchase a pup from a ethical breeder or adopt one from your local Animal Shelter or Pound.
