Because of her body being weak she could not handle giving birth to Ushio so before she gave birth the doctor already said that if she goes through the labor she wouldn't survive.

In general, Nagisa does not die in the series Clannad.
But if you continue to watch the after story, when Nagisa and Tomoya get married, Nagisa passes away giving birth to their daughter; Ushio. Very sad indeed~
But don't fret, for in the end, Tomoya is able to re-do his life...and his family is returned to him!

∙ 11y agoIn the world that has ended, it's shown that Tomoya Okazaki is the junk-gathered doll which that little girl made. In that world he is dreaming from the real world. He eventually collects mythalogical orbs ( shinning ball that if you catch it you get a wish) and Tomoya used the power of the orbs as a wish to have saved not only Nagisa Furukawa/Okazaki, but also little Ushio from death. So, Nagisa and Ushio died but when Tomoyo collapsed on he street with Ushio in his shoulder he enters the world that has ended and Nagisa and Ushio are saved. BTW, the little girl in the world that has ended was Ushio, but she eventually comes back to life.

What is clannad after story about?
"Clannad After Story" is the sequel to the "Clannad"anime . After Story is a path that opens in the original visual novel after all the other paths have been opened . Life continues on for Tomoya . A few months have passed since he confessed his love to Nagisa . Now entering the second semester , he continues to meet a variety of different people , expanding his own world in the process . Tomoya and Nagisa's relationship begins to enter a level of intimacy unlike before . Through his relationship with Nagisa and his various encounters , Tomoya begins to understand the meaning and importance of family . Unfortunately , as Tomoya and Nagisa begin their own family together they are faced with many hardships and challenges along the way . Picking up where season one left off , Tomoya Okazaki is a kind but depressed high school student who is trying to escape the pain of his troubled childhood. He has learned to help others in meaningful ways but he has a difficult time addressing his own problems. He's depressed, so he doesn't particularly like his life and if it weren't for his relationship with Nagisa he'd do little more than exist, floating through life with no direction or motivation. However, as his relationship with Nagisa progresses beyond high school it seems Tomoya may be doomed to repeat the tragic life and mistakes of his father. How does clannad end?
Basically the lights in the other world saved them. Tomoya collected a bunch of them throughout his life from helping a bunch of people (such as Miyazawa and his own father). When Ushio died, he lost all hope in life but he wished that Nagisa hadn't died while giving birth to Ushio and that Ushio was still alive. When he died (yes, he died in the last scene while clutching Ushio), he made that wish and was whisked back in time to when he first met Nagisa and was given a choice to whether or not meet her and have all those things happen. He decided to meet Nagisa and he fast forwarded to when Nagisa had given birth to Ushio and she had survived. Also, just to clear things up about the robot and the girl in the other world (they played a big part in Nagisa's revival), the girl is Ushio and the robot was Tomoya. Both sides of Tomoya (the robot and the one in the real world) were collecting a bunch of those lights and together, they combined all their lights and made one wish each. Tomoya made the revival wish while the robot wished that he and the girl (ushio) could go to the other world. Since they collected so many lights, their wishes came true. The girl was born into the other world as Ushio while the robot was reborn as Tomoya.
