You have to enroll in a oasas (Office Of Substance Abuse Services) certified treatment program, whether it be in-patient or out-patient. Upon successful completion the Director will sign and hand you or mail you a certificate of completion, which states that you have completed the necessary requirements set forth in your initial "Treatment Plan" which normally is decided within the first week of beginning your treatment. Each person's treatment plan sets forth certain goals that are specific to each individual, depending on the degree of abuse, the length of abuse, family situation, educational and/or vocational needs and financial status and other issues such as legal situations, child custody issues and many other obstacles that you need to deal with to "re-join society as a functional and productive member.

If this is for DMV to regain your driving priveleges, the same applies, but it is still up to the Driver Improvement Bureau to approve your application. Generally if your dwi record is not that bad it should'nt be a problem.
