I think what you're referring to is an irregularity that can occur in both men and women. It begins at the onset of puberty when your breasts fail to develop fully. This has several names; Tuberous Breasts, Constricted Breast Deformity, or Tubular Breasts. What you describe is not necessarily uncommon and there will most likely be several people in your school who are experiencing a similar situation.
There are several successful surgical procedures available to correct this anomaly. However, if there is a noticeable difference in your entire chest or rib cage, it may require a specialist plastic surgeon to address the correction. There are some plastic surgeons who have specialist training in this procedure.
If you live in the United Kingdom, or Canada, you should be able to have this matter resolved rather easily, as it's considered as a congenital deformity and therefore should be covered through the your National Health System. In the US, it is best to consult with a specialist in breast augmentation - specialising in this area.
Most importantly, it is not something in which you should be self conscious. There are numerous Bras and lingerie designed to provide shaping and forming. And as far as relationships are concerned, it is the whole you that your partner will love.
There is a number of websites you can visit which provide images of varying breast augmentations involving tuberous breasts. You might wish to enter simply "tuberous breasts" into a search engine and see what the results are, to see if they match what you've described herein.
Puffy nipples in men is a condition referred to as Gynecomastia. It can be caused by many things, but it is actually VERY common (affects the majority of men at some point in their life) and ranges from conical shaped puffy nipples to full on breast development. There are number of products on the internet trying to claim that they will cure or fix the condition, but the only way to permanently cure it if it does not go away is through surgery. An alternative is also to hide the condition under your shirt if you can not afford surgery or are still in puberty and think it might go away. There are tight undershirts that are made to keep the nipples from protruding through and shirt and also patches that can be put on the nipple if you are embarrassed. See the attached links.

They are just a natural condition. As you get older and your breasts are fully developed they will go away. Some women never get rid of them but to me that is actually a good thing.
