If you feel that the psychologist has a good plan of treatment and that your feelings are being validated, then it is a good idea to follow through on the plan and to do as advised. Second guessing the advice and then not following through (ie taking the meds) makes the whole exercise worthless.
Yes. A PsyD is a clinical psychologist. In fact, a Psy.D. is focused more on the clinical (treatment) aspects of psychology, whereas a Ph.D. is focused on research.
Any psychology, to be effective, must relate to a persons culture. When a person of a culture not the same as that of the psychologist seeks treatment, and the psychologist does not fully understand that persons culture, there may be a lesser ability to assist that person, and the distinct possibility of doing additional damage does exist.

No. The problem you are going to run into first, though, is paying for the psychologist. They aren't cheap. My mother was a psychologist, so I know, and she worked on the sliding scale so many will not use. By the way, I look at your categories and I understand. I had the same obsessions. As a psychologist he or she may undertake psychotherapy as a practice, as might a suitably qualified nurse, social worker, psychiatrist, pastor or clinical counsellor. Psychotherapy is an activity and a "psychotherapist" someone, of whatever profession, who undertakes that activity.
