Some university like VIT, amity provide including LPU provide counselling on the phone too.
The admissions office can get all that with your transcripts or if they ask for them SATs should be on there somewhere. You walk them in to admissions when you register.

I don't know the answer for Harvard specifically, but at most universities, you would call them up and ask for the admissions office. The admissions office probably can either send you the form, or tell you where to get it on line.
