Relapse prevention refers to a number of actions that, taken together, give an addict (or alcoholic -- same thing) the best chance at remaining clean and sober. It includes support groups, therapy, healthful living, nutrition, exercise, stress reduction, removal from old people, places and things until the person is solid enough to handle those stresses, development of some sort of spiritual (although not necessarily religious) practice, and education.
In my opinion, it should also include ongoing attendance at 12-step meetings. There are those who will disagree, but it has been both my personal and professional experience that those people have the best outcomes.
