Open your hood (Duh!) look to the left and you'll find 2 chrome tubes (chrome, silver, metal, w.e) the low pressure valve will accept the can of freeon's recharge valve, where as the high will not (thank God) and that's pretty much it. Look by your air filter for the 2 tubes going into the fire wall and follow them til you find the plug

The low side ac port on a 98 Pontiac Transport should be on the side of the drier. The drier should be mounted on the passenger side near the radiator.
I am assuming you are talking about the A/C. From the compressor, locate the large black hose comming from the A/C compressor. It goes into a small hard line, the valve on this hard line is the low side service port. The hose the comes from the A/C condencer is the high side service port.
Not sure what you mean by coolant port... Antifreeze can be added to the cap on the passenger side of the radiator if it is low and the reservoir on the driver's side should be filled to the line when cool

The low pressure service port is on top of the accumulator (the bottle) located on the passenger side of the engine compartment. It is next to the wiper fluid fill bottle near the grille. It is the SMALLER of the A/C service ports. The high port is on the driver's side. It has a big valve on a smaller diameter line. If you are filling with 134A you MUST fill at the low port...the one on the accumulator (bottle).
