Perhaps the most famed example was expressed in Mark Twain's "Huckleberry Finn" classic. Huck had been raised in 19th century America in which most all the churches taught that runaway slaves were bad, and any who helped them were bad, and even going to hell. Further, it was bad for whites to be friends with blacks.
However, when contemplating whether or not to turn in his friend "N*gger Jim", who was himself a runaway slave (or at least they believed he was), Huck mulled it over, thought about the "good" of turning him in, or the "right" thing of standing by his friend, and resolved to help his friend, though it send him to hell.
Twain was, of course, showing that the ethical system itself was flawed, that Huck - a dirty, illiterate homeless vagabond - had known more about right and wrong than the preachers preaching hate against the black.
Whenever you come across a situation in which there is a difference between "good" and "right", you may be sure that some error in the ethical system, or your application of it, is taking place.
