The radiator is not meant to stay full all the time. When over full the excess will flow to the overflow bottle. If that is not your problem, it is possible you have a head gasket leak and you are burning it or it is going to the crank case and you will have milky oil. All of these are conditions which will require service by a mechanic.

Perhaps plugged radiator - have it boiled out or replaced

It is worn and needs to be replaced. It is collapsing when the water pump draws coolant.

you have to drain the antifreeze/coolant so it would be wise.
No but it will give you alot more room to work and radiator fins are very delicate. If you insist on keeping the radiator in place cut out some thick cardboard and place it over the fins. I would most definitely remove it. I have replace the water pump, radiator, and thermostat in my 98 limited 5.9L
You would pour it in the radiator. Caution, stop leaks usually clogs heater cores.
Excess pressure is building up in the radiator probably from a blown head gasket and is exiting through the radiator cap and into the overflow bottle.
