More than likely your heater core is plugged, and will need to be replaced
The most common reason the heater is blows cold air on a 1999 Ford Winstar is a bad thermostat or blocked heater core.
the heater blows warm air when fan is on low but blows cold air when fan is turn to higher setting
Check to insure that the engine coolant is full and the thermostat is not stuck. Without the heated fluid from the engine the heater will always blow cold air.
some possibilities are. low coolant level, open thermostat, pluged heater core,
it is a heater. heat. it blows heat. warm air
Get the air out of your heater core.
Low coolant. If the coolant is low then the heater core is not able to transfer heat in to the car.

In most cases when a vehicle heater that has been working properly in the past begins to blow cold air the cooling system is low on coolant. It could also mean that your heater core is clogged. A good coolant system flush might take care of this, otherwise you may have to have it replaced.
You have a bad thermostat. It is a cheap fix (3-9$) in most cases, and fairly easy.
make sure the ac/heater knob or switch is actually is allowing the heater duct to open up for heat to enter the car.....it could be stuck on the ac position all the time and that's why your getting cold air instead of warm air...your heater its self is probably fine.
it may be a number of things.... the air blend doors get brittle and break then they fall and lay over the heater core blocking the hot air .... or the air blend door actuator may have broken which does not allow the blend doors to switch from hot to cold ..or cold to hot ..... or the heater core itself cold have gone bad
