It sounds like some kind of foreign matter has gotten into the fan housing. It could be a leaf that managed to get in via the fresh air inlet duct or a piece of loose insulation that got into the fan housing. Bottom line - you're going to have to get into it to clear it out.
As for why it blows only on the high speed you most likely will have a bad blower resister and for the noise, it could be a bad blower motor, bad blower wheel or trash that is stuck in the blower wheel or housing.......

A rumbling or humming noise with use of heat or AC that varuies with fan speed is usually caused by a blower motor that has a bad bearing. Replace the blower motor. Also, the next time you get the oil changed ask the mechanic to clear your AC evaporator drain tube and if you have a pile of leaves on your cowl, clean it off. If it's an irregular rattly type noise you may have debris such as leaves or raffle tickets stuck in the vents or ducts. If it's a sort of a thump you may have a rodent nest or dead rodent stuck in the squirrel cage. Remove the blower and dump out the nest/corpse as needed.
This buzzing noise is likely a faulty wheel bearing. The bearings are moving around randomly and need to be replaced.
Check the blower relay, it may be getting stuck..............

There is a Blower Motor Resistor Pack. Either the blower motor speed control switch itself is defective or the Resistor Pack is bad.
There is no provisions for lubricating the blower fan bearings If it is making noise then the bearings are probably worn out and you have to be replaced the blower motor The blower motor is visable from the engine compartment It is on the firewall probably on the passenger side Remove the battery cable Remove the electrical connectors Remove the four screws holding the motor to the firewall Remove the blower motor It may be stuck fast to the firewall and you will have to pry it loose

The most common cause for a 2002 Toyota Highlander's gas gauge to be stuck at three quarters of a tank is a faulty sending unit. The sending unit is designed to detect the current fuel level and relay it directly to the gauge.
