It could be a whole mess of things. But first check to see if the computer has any codes. You can go to a auto parts store like Advance Auto Parts, Shucks, Napa..etc and borrow a code reader for free. After that check for vacuum leaks, EGR Valve is a common problem in the 2004+ Rams, Throttle Position Sensor (if it's surging, that's what fixed mine), Cleaning your Throttle Body will help a lot.

sounds like you need to either turn up the idle some or get the carburetor rebuilt.
the idle jet in the carb is blocked and needs to be cleaned out and reset
Most likely the ignition module.
Idle Air Control Valve.
Sounds like the EGR valve needs to be cleaned or replaced.
My best guess is that the Idle Air Control (IAC) valve may need to be replaced.
need to have your idle checked, mine did the same thing, and adjusted the idle up and it stopped the problem, the only thing is you need a tack meter don't use the dash gauge, it may not be true
