Charleston, South Carolina had been a target for the Union ever since the bombardment of Fort Sumter in April of 1861. The Union continued to destroy the Rebel city with limited success. In 1863, there were several failed assaults on the city. A summary of the actions are as follows:1. The Union encountered failures in assaults on Charleston from July 10 and July 18, 1863;2. With these failures, the Union resorted to a more conventional approach to siege operations;3. This entailed three major artillery bombardments of unprecedented force;4. The first one lasted 17 days and nights from August 17 to September 2, 1863;5. Over 900 cannon shots were fired at and around Charleston;6. The second bombardment of 620 shots lasted 41 days;7. The third bombardment was tried some months later for 60 days; and8. Despite a virtual avalanche of artillery fire, supply lines between Charleston and its forts remained open till the end of the war.

Lots. There are a lot of variables to consider. What size artillery shell? What type of fuze? How close are the soldiers to each other? What sort of protection do they have? A large shell with an air burst over closely packed troops could easily kill dozens and wound dozens more. A small shell landing in a fox hole might only kill the soldier in that hole.
A bullet fired from a gun
The French built a fixed position of more than a dozen strong points at Dien Bien Phu, which was very far from main French bases, and was to be supplied by air strip. Dien Bien Phu ville is surrounded by higher ground. Giap brought artillery, by hand, up the hillsides and sited in on the air strip and strong points; artillery fire destroyed the targets one by one. The Vietnamese made one early assault, which failed, and went back to the artillery bombardment of many days. Eventually, casualties and destruction was enough that the French surrendered in the face of a major assault.
It depends, because there were many different types of bunkers. The average "pillbox" bunker (which fired the fearful MG42 machine gun) I would estimate to be no more than 6 feet tall in the interior (as if you're in it) and about 30 feet tall overall. The average casemate where the Germans fired their artillery from was probably around 12 feet tall. These may not be correct answers, but I'm just estimating.
