Check the oxygen sensor. It may be so clogged that it can't get accurate readings to send to the computer.
I have had a 1996 Chevy Silverado for 5 yrs. 200k+ miles and I am getting about 13 miles per gallon.

1500 and 2500 models are 26 gallon tanks, the 2500HD 3500 and 3500HD are 28 or 30 gallon i believe
