Take off the rear licence plate, there's a hole under there that is used to replace bulbs, or you may need to go from below the bumper all the way up.

The rear fog lights are accessable from under the car. You have to reach up and under the rear bumper.
Open the hood and then turn on the head lights. When the light opens you be able to reach in and twist the bulb out. You will certainly loose some skin off your hand when you do it!

If the aftermarket lights are not a factory option then the lights will need to be drilled into the bumper. Afterwards the lights will also need to be properly spliced into the existed wire harness.
Yes, but there will a much bigger gap between the bumper and the headlights than there was with the stock bumper. Additionally, when equipped with fog lights, the 01-02 bumper had different fog lights, so your new bumper may not come the new ones, meaning you'll have to either find a pair, or live with two great big holes in your bumper, that is of course assuming that the new bumper had fog lights.
