This depends on how worn the clutch components are, with some vehicles the clutch requires frequent adjustment to compensate for the friction plate wearing under usage. However if the clutch has been adjusted frequently and the 'slipping' remains, I'm afraid it's time to replace those internals.

You replace it. As long as you have the transmission out, you might as well put the clutch and pressure plate in also.
If it is the gearbox, then you have to dismantle it and replace faulty parts, such as a gear fork. This could also be a clutch problem.
