American Heart Association protocols do not recommend the use of pediatric AED pads on an adult victim because they deliver a lower energy discharge that will not usually be enough to shock the heart out of the dysrhythmia it is in (V-fib or V-tach). The mass of the adult body is enough that it will absorb the electrical discharge and not have the desired effect. It could also cause the heart to become "refractory" to defibrillation (the heart becomes more and more resistant to the shock) so that if an adult AED becomes available, it would no longer be effective. So if the pediatric AED is the ONLY one that is available and you do not anticipate that an adult AED will arrive within ten minutes of the onset of the heart attack, should you refuse to use the pedi one? Probably not. Remember that AHA's recommendation is just that -- a recommendation. You still do what you need to in order to save someone's life or at least give them a chance of survival. There is always a chance that the pedi AED might do the trick and convert the victim's heart rhythm.
