From everything I have been told and read, the three things you should be sure to have to go to Cuba as a Canadian are: your passport, a tourist visa (some agencies provide this in your package), and 25 Cuban pesos for the departure fee at the airport when you leave Cuba. The tourist visa can also be obtained from a Cuban consulate but can take a week to receive depending on the method you choose for applying (postal applications will obviously take more time).

A valid Canadian passport should be carried for all visits outside Canada. However, Canadians are only required to be in possession of valid official photo identification (such as a driver's licence) and proof of citizenship (such as a birth certificate) for travel to Jamaica. An onward or return ticket and proof of sufficient funds is also required.
