Yes, if you start from one of the rally point things it won't count, even if you have already finished the section before it.

to get the achievement you have to play it on normall dificulty or above and start from the begining of the mission not the check points

If you don't start the level from the beginning the skull won't appear. If you did start from the beginning i don't know.

No but to find the skull in that level you must start from the beginning of the level and be on either normal,heroic or legendary.
