Treatment really depends on the severity of the separation. If it is mild, avoiding strenuous activity, and sometimes bed rest will be ordered, either in the hospital or the home. Regardless of severity the health care professional will want to assess the fetal well being. However if it is a moderate to severe separation, you will remain in the hospital for observation and possible emergency C section. The treatment for moderate to severe abruptio which compromises maternal and fetal well being consists of IV fluids, supplemental oxygen, and an emergency cesarean birth. Depending on the HgB and Hct levels and blood loss, a blood or platelet transfusion may be indicated. Tocolytics such as mag sulfate are generally contraindicated in abruptio. However if in active preterm labor, the separation is mild, and blood loss is very minimal, a tocolytic might be used to postpone labor. This will depend on the provider, and only be used if the healthcare provider determines that the fetal development (Lungs) will benefit from delaying delivery in which the time will be utilized to administer steroid treatments to further develop the lungs. Lastly for drugs, an RH - mother will be administered the immunoglobulin RHoGAM to prevent mom from forming antibodies to future RH + pregnancies. NOTE: If abdominal pain, abdominal distention, light or heavy vaginal bleeding, increase in normal HR, fever, lightheadedness/hypotension, respiratory distress, alteration in normal cognitive status or decreased fetal movement is present, seek immediate medical attention, as abruptio placentae can cause hypovolemic shock in the mother, and possibly death of the unborn baby if not promptly treated.
