Still a preventable accident. Whether or not it's DOT reportable depends on exactly where it happened. If it happened on private property, it isn't, but can still be charged to the truck driver's DAC report.

None you want to drive for. Any trucking company that would hire someone without looking at their DAC report would also cut corners on maintenance, demand you run illegal logs, and other abuses.
