The same way that they always have, and still do today. A Bishop is ordained or consecrated by another Bishop, usually by three other Bishops. All Catholic Bishops are chosen by the Holy Father from the ranks of priests.

Francis Aloysius Sullivan (From Apostles to Bishops: The Development of the Episcopacy in the Early Church) says the consensus of scholars, both Catholic and non-Catholic, is that at first, larger Christian communities were led by a college of presbyters, which were sometimes known (collectively) as 'bishops', or episcopoi.

Sullivan says that the monoepiscopal system of a single bishop arose in the second century, initially in Syria and Asia Minor, then spreading to Corinth and Rome, then other centres. The earliest supervisory bishops were elected with involvement by the laity.

Gradually, bishops were appointed. During the Middle Ages, depending on the relative power of the pope and the secular rulers, bishops were appointed by either the sovereign or the pope. The appointment of bishops became a lucrative business for many popes during the Middle Ages and Renaissance period, as bishoprics were sold, often to the highest bidder.

By early modern times, it became a requirement that bishops of the Catholic Church were chosen by the pope, and consecrated by another bishop.
