securely and safely jack up rear of van supporting both sides. A vehicle falling can Kill pay particular attention to how you support vehicle, make sure it's on a level surface, won't roll etc. If you can't safely support the rear axle assembly off the ground, hire a professional.
pull cover, drain gear oil, find holding bolt on pin, 5/16 head, remove, remove pin. push axle in and c-clip will fall out. don't push vehicle off jack stands or supports, falling vehicles can maim or kill. Remove axle, now you see bearing, check axle for damage from bearing, replace worn parts as necessary. Don't forget, lifting a vehicle off the ground can be dangerous, if you can't do it safely hire a professional.

I have a 1997 Astro and there are actually three (3) oxygen sensors on it. One on each down tube of the Y before the catalytic converter and one between the catalytic converter and the muffler back toward the rear axle. The one the driver's side Y isn't bad to change. The one on the passenger side is impossible and the one to the rear is tough as well.

You must remove the axle to replace the backing plates. It is job for the professional do-it-yourselfers with all the tools.
