There are extension poles for just this purpose available at lowes and Home Depot. When those poles will not work for whatever reason the only thing left to do is to get up there and change the bulb, fan, or fixture whichever the case may be.
One ladder that is most useful is a 21' A frame ladder from Little Giant. It conforms to meet the demands of many interior spaces and depending on the job can be much better than transporting, setting up, and then disassembling scaffolding to reach high out or reach spaces. It is still quite large and heavy but it takes a two man crew to deal with it.
