There are two ways to address a female doctor and her husband formally: * If it's a friendly get together then you could address them to each guest as 'This is Dr. Mary Jones and her husband Jack. * If it very formal then introduce them as Mr. Jack Jones and his wife Dr. Mary Jones.
Just as you say, i.e. Dr. and Mrs. John Greene, unless Mrs .Greene is also a doctor. Then they are Doctors John and Mary Greene.

The envelope should read: Mr. John Doe & Dr. Jane Doe. If you don't know her first name then put: Mr. & Dr. John Doe.
The address should read (an example): Dr. John Doe and Mayor Jane Doe.

You can address a retired doctor and his wife as "Dr. and Mrs. (Last Name)" when speaking to them formally. If you are on more familiar terms, you could simply address them by their first names.

If you are addressing a married couple who are both doctors you can address them two ways. You can address them by either stating Drs. Jack and Jill Smith or Dr. Jack Smith and Dr. Jill Smith.

When addressing an envelope to the wife of a doctor who is not a doctor herself then and he is Dr. John Doe, then address the envelope to her as Mrs. John Doe or Ms. (Mrs.) Jane Doe.
