There are no effective spells that can change your gender, race, or species.

A more feasible way:

The generally accepted practice is to require the patient to undergo a psychological evaluation, undergo hormone replacement therapy (taking estrogen or testosterone; may include chemical castration), and may or may not undergo sex reassignment surgery (often referred to as a "sex change" by people not knowledgable about transsexualism; involves reshaping the genitals to achieve a look more like that of a born female or male).

Some people undergo additional aesthetic surgeries. Male-to-female transsexual people (those designated male at birth) sometimes have surgeries to make them appear more feminine, like facial feminisation or breast augmentation. The patient may require several others to fully shape the body (ie, chin shaving, nose jobs, rib removal, etc). Female-to-male transsexual people (those designated female at birth) often have breast removal/reduction to create a more masculine-shaped chest.

Sex "change" is not what most people think it is. A man or a woman desiring a "sex change" can never wind up 100% changed to the other sex. The operations themselves are merely external to create a facsimile of the desired sex. It is only a partial change.

Males and females do not have the same genetic code. They are conceived and born with either XX or XY chromosomes and that chromosome "stamping" can never be altered by surgery or drugs. We all remain genetically the same as when we were born. However, if you were born with the wrong body and chromosomes for who you have really been inside your entire life, then hormones and surgery will be as close as you will ever get.
